dosing arithmetic — critical reminder about tirzepatide dose equivalence across formulations: Mounjaro KwikPen and compounded tirzepatide are the same…

Ongoing cost tracking for dosing arithmetic — transparency for the community:

Monthly medication cost: $123
Supplies (syringes, BAC water, alcohol swabs): ~$11/month
Janoshik testing: ~$13/month (amortized quarterly)
Lab work: ~$43/month (amortized quarterly)
Total: ~$193/month

Compare to brand Wegovy at $1263/month without insurance. Compounded is roughly 90% cheaper for what I have found to be clinically equivalent medication.

Agreed, and it is worth saying that rounding to the nearest whole unit is usually acceptable at maintenance doses and is not acceptable at the bottom…

Saving this. It is the first explanation that did not require me to already understand it.

For anyone reconstituting dosing arithmetic peptides: the water-to-powder ratio determines your concentration. Here's my reference:

Vial SizeBAC WaterConcentration0.25mg Dose
5mg2mL2.5mg/mL10 units
5mg2.5mL2mg/mL12.5 units
10mg2mL5mg/mL5 units
10mg3mL3.33mg/mL7.5 units

More water = easier to measure small doses but vial runs out faster. Find the balance that works for your dose.

I reconstituted a 10mg vial with 2ml of bacteriostatic water and I want someone to check my unit arithmetic before I draw anything, because I keep…

Just received my dosing arithmetic order — packaging review:

  • Outer box: plain brown, no medication identifiers (discreet) ✅
  • Inner insulation: Styrofoam with 4 gel ice packs ✅
  • Temperature: vials were cold to touch on arrival ✅
  • Labeling: lot number, BUD, compound name, concentration ✅
  • Included: COA, reconstitution instructions, dosing card ✅

This is what professional compounding pharmacy shipping looks like. If your order arrives without these basics, reconsider your vendor.

Moderator note: the sourcing question belongs in the vendor section and has been split out. Tagging this one for the weekly digest.
