Pharmacist here. Let me clarify the current state as of June 2026:

Mounjaro Savings Card (for T2D indication with commercial insurance):

• Eligible patients pay as low as $25 per 1-month or 3-month fill
• Maximum savings of $150 per 1-month fill or $450 per 3-month fill
• Valid for all dose levels (2.5mg through 15mg)
• Requires commercial insurance with coverage for Mounjaro
• Card is registered per-patient, one card per person

Zepbound Savings Card (for obesity indication with commercial insurance):

• Similar structure but separate enrollment
• $25 copay for eligible patients
• Lilly has been adjusting the terms periodically so check the website

Important: If your insurance doesn't cover the drug at ALL (prior auth denied, not on formulary), the savings card typically won't apply. You need at least partial insurance coverage in most cases.

All doses cost the same at the manufacturer level, so the savings card value doesn't change with dose.

My experience with the savings card has been... inconsistent. Last month my pharmacy charged me $25, this month they're saying $175 because of "benefit coordination issues." I'm calling Lilly's support line tomorrow.

Has anyone else had their pharmacy mess up the card processing? I'm at CVS and they seem confused every single time.

MarkLI_maint — switch to a dedicated specialty pharmacy if you can. I had the same issue at Walgreens and when I moved to Alto Pharmacy, they processed the card correctly every time. CVS and Walgreens seem to have the most issues with manufacturer copay cards in my experience.

What about those of us whose insurance covers Mounjaro but at Tier 4 non-preferred? My copay before the savings card is $475/month. The savings card knocks it down to $325. That's still a LOT of money.

Anyone had luck getting a tier exception? My endo submitted one twice and got denied both times.
