This is eye-opening. So basically my PCP would think I'm "cured" based on A1C alone, while my endo sees that the underlying metabolic dysfunction is still very much present. That's actually a huge deal for treatment decisions — like whether/when to come off sema. Anyone know if HOMA-IR typically improves over time on GLP-1s with continued weight loss?

Yes, HOMA-IR does typically improve with sustained weight loss, but the timeline varies. Visceral fat loss is the biggest driver — that's the fat around your organs that drives insulin resistance. Some data: - 5-10% body weight loss: HOMA-IR typically drops 20-30% - 10-15% body weight loss: HOMA-IR drops 30-50% - >15% body weight loss: HOMA-IR often normalizes (approaches <2.0) - Exercise independently reduces IR, additive with weight loss But it's not linear. Some people hit normal HOMA-IR at 12% weight loss. Others need 20%+ before insulin sensitivity fully normalizes. Genetics play a role — some people are just more prone to IR at any weight. The important thing is trending it over time. Ask your endo to track HOMA-IR quarterly. Watching it improve is incredibly motivating and gives you a metabolic endpoint beyond just scale weight.

Practical suggestion: ask your endo to write a "to whom it may concern" letter summarizing their recommended monitoring protocol for you as a GLP-1 patient. You can give this to your PCP. Most PCPs are happy to defer specialist monitoring — they have 15 minutes per visit and 2,000 patients. They genuinely appreciate when a specialist says "here's exactly what I want monitored and why." Also: many endo offices have a patient portal or will fax results. Request that both offices send you complete copies of all labs. You should have your own master file. I use a Google Drive folder with PDFs organized by date. Being your own health data coordinator is unfortunately necessary when you have multiple providers.

Taking everyone's advice. Going to: 1. Ask my endo for a monitoring protocol letter for my PCP 2. Start maintaining my own lab tracking spreadsheet 3. Request both offices send me all results directly 4. Defer metabolic lab interpretation to my endo 5. Keep PCP for general health, acute issues, and medication management The HOMA-IR education alone was worth this post. I had no idea my insulin was that elevated or what it meant. A1C really can give a false sense of security.
