PLEASE let my awful experience be a warning to everyone on GLP-1 medications. I've been on tirzepatide 10mg for 6 months and honestly wasn't great about hydration. I was eating maybe one meal a day, drinking coffee and maybe 3-4 glasses of water. Two weeks ago I ended up in the ER with the worst pain of my life — a 4mm kidney stone. The urologist directly asked if I was on a GLP-1 medication and when I said yes, he said he's been seeing a dramatic increase in kidney stones in GLP-1 patients. The mechanism is simple: you eat less, you drink less, your urine becomes concentrated, and stones form. I am begging you all — DRINK WATER. More than you think you need.

Urology NP here and I can confirm we are absolutely seeing more kidney stones in the GLP-1 population. My practice has tracked a noticeable uptick over the past 18 months that correlates with the explosion of GLP-1 prescriptions. The pathophysiology is straightforward: chronic mild dehydration leads to supersaturated urine, which leads to crystallization, which leads to stones. Compounding factors specific to GLP-1 patients: reduced fluid intake due to appetite suppression, potential fluid losses from nausea/vomiting/diarrhea, increased protein ratio in diet (high protein/low overall calories concentrates waste products), and the medication itself may affect renal handling of certain minerals. Minimum recommendation: 2.5-3 liters of fluid daily, and your urine should be pale yellow to clear at all times.

Two kidney stones in 8 months on semaglutide. First one passed on its own (barely — worst 6 hours of my life), second one required lithotripsy. My nephrologist put me on a strict hydration protocol: minimum 100oz of water daily, 4oz of lemon juice daily (citrate prevents calcium stones), limited oxalate foods (spinach, almonds, chocolate), and monitoring 24-hour urine collections every 3 months. I now carry a 40oz water bottle literally everywhere and track my intake in an app. Not getting a third stone.

The lemon juice thing is interesting — my urologist mentioned that too. Something about citric acid binding to calcium and preventing it from crystallizing. I've been adding fresh lemon to all my water now. Also, I had no idea that high protein with low overall calories could concentrate waste products. I've been doing protein shakes as my primary food source which is probably exactly the wrong approach for kidney health without adequate water.

Renal dietitian here. The protein shake issue is a great point. When you consume protein, your body breaks it down and produces urea, uric acid, and other waste products that must be excreted by the kidneys. If you're consuming 80-100g of protein daily but only eating 1000-1200 total calories and not drinking enough water, you're essentially concentrating all that metabolic waste. It's not that high protein is bad — it's that high protein REQUIRES high fluid intake. Rule of thumb for GLP-1 patients: for every 10g of protein you consume, drink at least 8oz of water. And yes, lemon water is excellent for stone prevention — aim for the juice of 2 lemons per day in your water.
