Can confirm the pattern DebRD_ATL describes. Worth adding the genuine exception, because it is real and narrow: a change made for an identified patient where the prescriber determines it produces a significant clinical difference for that patient. A grid of fixed doses offered to everybody is not that, whatever the intake form says.

Correct as far as it goes. The part it does not cover is what to do when the honest answer is "not enough data", which is more often than anyone likes.

Clinical perspective, offered as context rather than as advice. Most of what circulates confidently in this community traces back to one summary of one study, and the qualifier was dropped somewhere in the third retelling.

Worth separating that from compounded supply, which this thread keeps folding into the same question. They behave differently and the advice does not transfer.
