Can we talk about something that NO ONE mentions? My knee pain is gone. GONE.

I've had bilateral knee pain since my late 30s (I'm 47). Diagnosed with early osteoarthritis at 42. Was taking 800mg ibuprofen 2-3x daily. Ortho was talking about eventual knee replacement.

5 months on semaglutide, down 38 lbs, and I haven't taken ibuprofen in 6 weeks. I can go up and down stairs without wincing. I walked 4 miles yesterday. FOUR MILES. I couldn't do one mile in January.

Is this just the weight loss? Or is there something else going on? Because the improvement seems disproportionate to the weight lost.

You're touching on something that's getting a LOT of research attention right now. The joint pain improvement is likely from both weight loss AND direct anti-inflammatory effects of semaglutide.

Weight loss effect: Every pound of body weight translates to approximately 4 pounds of force across the knee during walking.[1] So your 38 lb weight loss has reduced the mechanical load on your knees by roughly 152 lbs of force per step. That's enormous.

Anti-inflammatory effect: Independent of weight loss, GLP-1 receptor agonists have demonstrated anti-inflammatory properties. Semaglutide reduces CRP, IL-6, and TNF-α — all of which are elevated in osteoarthritis and contribute to cartilage degradation and synovial inflammation.[2]

There's also emerging evidence that GLP-1 receptors are expressed on chondrocytes (cartilage cells) and synoviocytes (joint lining cells), suggesting a direct protective effect on joint tissue that's separate from weight loss or systemic inflammation reduction.[3]

So yes — the improvement likely IS disproportionate to the weight loss, because you're getting a mechanial benefit PLUS a pharmacological anti-inflammatory benefit PLUS a possible direct chondroprotective benefit. Triple win.

This!!! My rheumatologist actually brought this up at my last appointment. She said she's seeing dramatic OA improvement in multiple patients on GLP-1 agonists and it's "the talk of the specialty." She thinks within 5 years there will be a formal indication for GLP-1s in OA management.

e experience here but with my lower back. I have L4-L5 degenerative disc disease and the pain was constant — like a 5-6/10 every day. 7 months on sema, down 44 lbs, and I'm at maybe a 2/10 on most days. My pain management doc reduced my gabapentin dose for the first time in 4 years.

It's not just knees. The anti-inflammatory effect seems systemic.

My hip pain improvement has been so dramatic that my orthopedic surgeon asked what changed. When I told him semaglutide, he said "I've been hearing that a lot lately." He's now referring some of his obese OA patients to weight management before scheduling surgery. That's a sea change in orthopedic thinking.
