Distribution matters less than total but it is not nothing.

DEXA scan update relevant to protein intake:

Scanned at months 0, 6, and 12. Here are the body comp numbers:

BaselineMonth 6Month 12
Total mass249 lbs239 lbs216 lbs
Fat mass119 lbs69 lbs51 lbs
Lean mass157 lbs144 lbs153 lbs
Body fat %42%30%25%

Lean mass preserved — actually GAINED 1-2 lbs thanks to progressive overload + 1.4g/kg protein. The protein intake discussion needs to include body comp data, not just scale weight.

A narrower follow-up, since the general answer is now clear:

How people are hitting a protein target on a genuinely suppressed appetite, because volume is the binding constraint rather than willingness?

Coming at wendy_avl’s question from a different direction. The version of this that has an answer is narrower than the version being asked. Narrow it and it becomes tractable; leave it broad and the thread will produce nine confident and incompatible replies.

Closing the loop on my own question.

Front-loading was the answer. Same total intake, same drug, but two thirds of the protein before midday and I am hitting the target most days now.

Agreed on the target, with the caveat that reference weight and current weight are different numbers. Using current weight at the start of a large loss produces a target that is both unachievable and unnecessary.
